Referring Physicians

Please download our Physician Referral Form and fax it to us to schedule a patient consultation with a vitreoretinal specialist at one of our locations in Durango, Farmington, or Cortez. If you would like to discuss a particular patient before referring them to us, please do not hesitate to call.

If your referral is urgent, call our office directly after faxing the referral form to expedite the timing of the patient's evaluation.
